Form #: WAT142 Applications and Forms Revised: 11/1/21

November 1, 2021 -- The Nebraska Department of Environment and Energy (NDEE) sent out the Clean Water (Wastewater) Needs Survey to Nebraska communities, Natural Resources Districts, consultants and engineers.

The Needs Survey is sent out annually to every Nebraska community in order to understand the state’s wastewater infrastructure or nonpoint source control (such as watershed improvement projects) needs. The state evaluates these Needs Surveys to establish communities’ priority points for the Intended Use Plan of the Clean Water State Revolving Loan Fund. The Needs Survey filled out in 2021 will be used in the 2023 Intended Use Plan. For more information go to:

Wastewater and Drinking Water Financial Assistance Programs

Filling out this survey in no way commits a community to undertaking a wastewater project or signing a Clean Water State Revolving Fund loan. For more information, refer to the NDEE “Cover Memo” above.

NDEE developed the Assessing Wastewater Infrastructure Needs (AWIN) program to assist struggling communities in Nebraska to better afford, maintain and operate wastewater infrastructure projects. For more information, and to find the AWIN Sustainability List and Individual Community Datasheets to use when filling out the Needs Survey go to:

AWIN: Assessing Wastewater Infrastructure Needs

NDEE is requesting that communities fill out the form and send it to NDEE, postmarked by
